Thank you for being a member of the SQCN Team. You may not be aware of the importance your participation makes to your SQCN patients and the network in general. SQCN is a physician led Clinically Integrated Network (CIN). It’s organized to bring community providers together to work collaboratively with colleagues to improve the quality, efficiency, and access of care. The SQCN network delivers healthcare, social, and pharmacy services to meet the clinical and financial metrics of the network while boosting practice capabilities and staffing, when needed.

Insurance companies and employers in our communities look to SQCN to better manage the care of their beneficiaries—or their employees and family members—by offering a network of providers who:

• Are dedicated to the principles of a CIN (physician led, data sharing, quality, and efficiency).
• Work with professional colleagues and support staff to coordinate care.
• Use network data and analytics to identify improvement opportunities.
• Identify and share best practices across the network.

Our network depends on each of the SQCN providers to engage, manage, and deliver care that will demonstrate higher quality and improved efficiencies for the patients, employers, and insurers we serve.
